How can I apply for CareCredit?
You can apply online or download and print the application and take it to your healthcare provider. The application is quick and easy, and after it’s submitted, you’ll instantly learn if you’re approved.
What kind of payment options does CareCredit Offer?
CareCredit offers a full range of payment options. We offer 6, 12, 18, and 24 month plans where you can avoid paying interest if you make your minimum monthly payments and pay the full amount due by the end of the promotional period. Interest will be charged to your account from the purchase date if the promotional balance, including optional charges, is not paid in full within 6, 12, 18 or 24 months.. Minimum monthly payments are required.
For patients and clients with higher treatment fees who would prefer more time to pay with lower monthly payments, CareCredit offers longer payment options with a reduced, fixed interest rate. The fixed monthly payment amount is based on repayment over a 24, 36, 48 or 60- month period. Purchases of $1,000 or more are eligible for a 24, 36 or 48-month offer and purchases of $2,500 or more are eligible for a 60-month offer. Not all practices offer all CareCredit financing options, so check with your provider.
You must make at least the fixed minimum monthly payment on or before the due date each month to keep your account in good standing. You can always pay more than the minimum payment. To find out which plans are available in your provider’s practice, be sure to speak with the office manager or financial coordinator.

What is your appointment cancellation policy?
Due to the attention we give every patient a 48 hour notice is required to cancel an appointment. To cancel and reschedule your appointment please give Dr. David’s office a call at 770.922.6149. We do not double or triple book, like so many offices. We schedule ONE patient at a time and our patients rarely, wait more than 5 minutes, to be seated. Our confirmation phone calls, texts and emails are sent out 4 days in advance of appointments. We are certainly empathetic for emergency cancellations but request, for the respect of patients on our waiting list, to please call immediately if you need to reschedule.
Will you accept my insurance?
We will submit ANY dental insurance. Some plans require treatment by contracted clinics only. As long as your plan has “out-of-network benefits,” you will be able to use them in our office.
